Konami Penalty Guidelines wrote:

Basically, what this means is, since Hero failed to notice that the gamestate was in disrepair, and allowed the gamestate to continue, he is also at fault. This happens all the time in real competition. Since Hero admitted defeat and stepped away from the duel, its the equivalent of signing the result card, and then noticing afterwards what had happened. If you call a judge over after you already signed it, they will point out what i have bolded.
You can no longer recreate the gamestate of that duel because everything got pulled from the field. if you had noticed it when it was happening, you could have rewound and corrected it, but since you accepted the loss, there's nothing we can do to put the duel back together now, and as is mentioned, a duel or match will not be replayed over irrepairable gamestate
